What this policy covers
This policy explains how Sylvia A. Gisi, M.D. — Family Practice handles information you share through this website and through our text messages. Your medical records are protected separately under federal HIPAA rules and California law, and are covered by our Notice of Privacy Practices. Ask at the front desk for a copy.
Information you give us
When you send an appointment request, we receive what you enter in the form:
- Your name and date of birth
- Your phone number, cell phone number and email address
- How you prefer to be contacted, and your preferred date and time
- A short description of why you would like to be seen
Please don't include detailed medical information or anything urgent in the form. When you call or text the office, we receive your phone number and your message.
Information collected automatically
This website does not use advertising cookies or analytics tracking. Like any website, our hosting provider may record basic technical information, such as your IP address and browser type, to keep the site secure and working. Our pages load fonts from Google, which receives your IP address when a page loads.
How we use your information
We use your information to respond to your request, schedule appointments, send reminders, answer your questions and run the practice. We do not sell or rent your information to anyone.
Who we share it with
We share information only with service providers that help us run this website, our phone system and our text messages, and only so they can provide that service. We also disclose information when the law requires it.
Patient portal
Our patient portal is operated by a separate company. Your use of the portal is covered by that company's own privacy terms.
SMS and text communications
By providing your cell phone number and opting in, you consent to receive text messages from Sylvia A. Gisi, M.D. — Family Practice. Message frequency varies. Message and data rates may apply. You can opt out at any time by replying STOP to any message. Reply HELP for help.
SMS data privacy
When you opt in to text messages, we collect and store your phone number and your consent status so we can send appointment reminders, office notices and replies to your questions. We do not include test results or other detailed medical information in text messages.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent are not shared with any third party, except the service providers that deliver our messages, such as our phone system provider and wireless carriers.
Children
Appointment requests for a patient under 18 should be sent by a parent or guardian.
Keeping information secure
We use reasonable safeguards to protect the information you send us. No transmission over the internet is completely secure, so for anything sensitive please call the office or use the patient portal.
Changes to this policy
If we change this policy, we will post the new version on this page and update the date at the top.
